Hello, this is Dr. Cha-Young Kang, the director of Nose Lab Clinic.
Today, I’m sharing a case of a patient who came to us troubled by a wide, bulbous nose (commonly referred to as a "round nose"). Through functional and aesthetic corrections, we achieved not only a refined nasal structure but also a harmonized overall facial appearance.
1. Key Concerns
Wide, bulbous nasal tip
Nasal obstruction
Prominent protruding upper lip (convex profile)
2. Preoperative Analysis
Design Consultation: Frontal View, Right Oblique Side View, and Side View Photos
A. Front View:
Wide nasal tip
Undefined boundary between nasal tip and alar cartilage
Slightly deviated nose
B. Side View:
Low nasal bridge
Protruding upper lip (convex profile)
Subtle dorsal hump creating irregular contours
C. Nasal Base View:
Asymmetrical nostrils
Flattened columella
Widely spread nasal tip
Design Consultation: Left Oblique Side View, Side View, and Nostril View Photos
Functional Assessment:
Diagnostic tests revealed left-sided septal deviation and enlarged inferior turbinates on both sides, leading to a diagnosis of septal deviation and hypertrophic rhinitis.
3. Surgical Plan and Execution
A. Functional Corrections:
Septoplasty to correct the deviated nasal septum.
Radiofrequency turbinate reduction to address hypertrophic rhinitis and improve airflow.
B. Structural Realignment:
Lateral osteotomy to narrow the nasal bridge and correct deviations.
Augmentation rhinoplasty using silicone implants for a smooth and elevated nasal bridge.
Septal extension with autologous rib cartilage to stabilize and lengthen the nose.
C. Tip Refinement:
Partial removal of excess soft tissue to reduce nasal tip bulk.
Repositioning and suturing of alar cartilage to narrow the nasal tip and improve definition.
Columella augmentation with rib cartilage grafts to lower the columella and enhance the nasolabial angle.
4. Results
A. Front View:
Before (Left) / Immediately After (Right)
A refined, narrower nasal tip
Clearer definition between nasal tip and alar cartilages
A straighter nasal bridge
B. Side View:
Before (Left) / Immediately After (Right)
Enhanced dorsal height at the bridge for a balanced profile
Smoothed dorsal hump for a natural contour
Improved nasolabial angle, creating a softer and more feminine silhouette
Before (Left) / Immediately After (Right)
C. Nasal Base View:
Before (Left) / Immediately After (Right)
Balanced, symmetrical nostrils
Natural and subtle projection of the columella
Harmonious alar cartilage alignment
Overall Outcomes:
Enhanced facial symmetry and balance
Improved breathing function and resolution of nasal obstruction
Boosted confidence with a refined and natural appearance

5. Closing Thoughts
Rhinoplasty is not just about reshaping the nose; it’s about enhancing the balance and harmony of the entire face. This case highlights how addressing multiple concerns—both functional and cosmetic—can dramatically transform a patient’s overall appearance.
Key Takeaways:
Functional improvements (like alleviating nasal obstruction) are as crucial as aesthetic corrections.
Refining the nasal tip and bridge can bring remarkable changes to the overall facial contour.
A customized approach ensures each patient receives results tailored to their unique facial structure and goals.
