Achieving a Balanced Look: Correcting Wide and Bulbous Noses_Rhinoplasty Korea

Today, I’m sharing a case of a patient who came to us troubled by a wide, bulbous nose (commonly referred to as a "round nose"). Through functional and aesthetic corrections, we achieved not only a refined nasal structure but also a harmonized overall facial appearance.


1. Key Concerns

2. Preoperative Analysis

A. Front View:

  • Wide nasal tip

  • Undefined boundary between nasal tip and alar cartilage

  • Slightly deviated nose

B. Side View:

  • Low nasal bridge

  • Protruding upper lip (convex profile)

  • Subtle dorsal hump creating irregular contours

C. Nasal Base View:

  • Asymmetrical nostrils

  • Flattened columella

  • Widely spread nasal tip

Functional Assessment:

  • Diagnostic tests revealed left-sided septal deviation and enlarged inferior turbinates on both sides, leading to a diagnosis of septal deviation and hypertrophic rhinitis.


3. Surgical Plan and Execution


A. Functional Corrections:

  • Septoplasty to correct the deviated nasal septum.

  • Radiofrequency turbinate reduction to address hypertrophic rhinitis and improve airflow.

B. Structural Realignment:

  • Lateral osteotomy to narrow the nasal bridge and correct deviations.

  • Augmentation rhinoplasty using silicone implants for a smooth and elevated nasal bridge.

  • Septal extension with autologous rib cartilage to stabilize and lengthen the nose.

C. Tip Refinement:

  • Partial removal of excess soft tissue to reduce nasal tip bulk.

  • Repositioning and suturing of alar cartilage to narrow the nasal tip and improve definition.

  • Columella augmentation with rib cartilage grafts to lower the columella and enhance the nasolabial angle.


4. Results
