Asymmetric Nose and Deviation Correction _ Closed Rhinoplasty Korea

Hello, this Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today, we will introduce a corrective case of a patient with severe nostril asymmetry and nasal deviation.


This patient had undergone rhinoplasty at another hospital 2 years before but had not been able to achieve satisfactory results, and developed various issues, therefore she visited our hospital.

Frontal view:

  • Wide nasal dorsum

  • General deviation of the nose

  • Nose tip tilted to the left side

  • Severe nostril asymmetry

  • Excessively lowered columella which gives the nose an "arrow-shaped" tip.


Side profile view:

  • High nose starting point and long nose

  • The low columella makes the nostrils excessively visible.


45-degree angle view:

  • An overall long and droopy nose

Surgical Plan

After determining the patient's condition, and listening to her requirements, we developed the following surgery plan:


  • Medial and lateral osteotomy to correct the width of the nasal dorsum, and the nasal deviation.

  • Septal reconstruction using autologous rib cartilage.

  • Septoplasty

  • Inferior turbinates submucosal resection surgery using a high-frequency laser.

  • Curved nasal bridge shape (using silicone as necessary)

  • Correction of the wide nasal dorsum into a slimmer shape.

  • Straightening correctin of the deviated nose tip.

  • The nose tip that was tilted to the left side was fixed to the center.

  • Great improvement of the nostril asymmetry.

  • The "arrow-shaped" nose appearcance disappeared, and the nose took on a more natural-looking shape.

  • An additional correction of the nasal hump resulted in a soft curved nose shape line.

  • The long and droppy nose was adjusted to a better fitting height and lenght.

  • The overall droopy parts of the nose were corrected into a smooth shape.

  • Balance enhancement achieved by the overall slimming of the previously wide nasal dorsum.

Overview


This case complements the shortcomings of previous surgeries and simultaneously resolves complex problems. It is particularly significant that the two major problems, of nostril asymmetry and a crooked nose, were successfully corrected through closed rhinoplasty.

Septal reconstruction using autologous costal cartilage played a key role in securing the structural stability of the nose. This allowed for simultaneous resolution of multiple problems, including droopy nose tip, deviated nose, and asymmetry of the nostrils.

The new osteotomy technique allowed us to correct the asymmetrical results of the previous surgery, narrow the width of the nasal dorsum, and create the slim shape the patient wanted. In addition, by delicately refining the nose bridge line to create a soft curve, we were able to complete a natural yet beautiful nose shape without additional implants.

Through correction of septal deviation and resection of the inferior turbinates, significant improvements were also achieved in terms of functionality. It is expected that the resolution of nasal obstruction problems will also contribute to improving the patient's quality of life.

By performing all of these procedures using a closed rhinoplasty approach, we were able to shorten the postoperative recovery period and complete the surgery without leaving any external visible scars, which have greatly contributed to increasing patient satisfaction.

Conclusion

I hope this case provides hope to those who are concerned about nostril asymmetry and nasal deviation. I think this is a good example that shows that even if you are not satisfied with the results of your previous surgery, you can still get the results you want with an appropriate reoperation.
