Hello, this is Dr. Cha-Young Kang from Nose Lab Clinic.


Today, I’d like to share a case where we successfully corrected a bulbous nose while improving nasal congestion. The goal was to create a more defined and structured nasal shape while addressing functional breathing concerns.


1. Initial Observations


a. Frontal View

  • Wide, flat nasal structure

  • Low nasal bridge

  • Lack of overall definition and projection


b. Side Profile

  • Blunt, rounded nasal tip

  • Mild protrusion of the mouth (suggesting nasal profile imbalance)

  • Under-projected nasal bridge


c. Nostrils & Symmetry

  • Asymmetrical nostrils

  • Overly broad and rounded nasal tip

  • Lack of proportional harmony

2. Patient’s Requests


  • Improve nasal congestion

  • Enhance nasal definition

  • Slim down and refine the bulbous nasal tip

  • Achieve a more structured yet natural appearance


3. Surgical Approach


Step 1: Bulbous Nose Correction – Refining the Skin & Soft Tissue

  • Reduction of subcutaneous tissue thickness to improve definition

  • Controlled soft tissue removal to refine the nasal tip contour

  • Initial narrowing of the tip width


Step 2: Structural Correction – Cartilage Repositioning

  • Repositioning the alar cartilages for improved structural support

  • Precise suturing techniques to refine the nasal tip shape

  • Stabilization of the newly formed nasal tip framework


Step 3: Projection & Balance – Enhancing Nasal Height & Definition

  • Nasal tip projection adjustment to improve three-dimensionality

  • Fine-tuning the nasal contour for a naturally slimmer appearance

  • Final adjustments to ensure balance with the rest of the facial features

This case was particularly focused on bulbous nose correction while ensuring that the results remained natural and well-balanced. A structured, three-step approach was used to refine the tip, reposition the cartilage, and enhance the nasal projection.


Among these, Step 3 (adjusting nasal projection and definition) was the most critical—this final stage ensured that the newly refined nasal tip appeared naturally slim and well-contoured rather than appearing surgically altered.


Additionally, functional improvements were made to relieve nasal congestion, ensuring that the patient experienced both aesthetic enhancement and better breathing.
