Today, I’d like to share the case of a patient who sought surgery to address columella depression and a "piggy nose" appearance. This case highlights the transformative power of precision rhinoplasty in achieving balance and harmony with the face.


1. Surgical and Procedural History


  • 1 year ago: Nose filler (dissolved prior to surgery)

  • 2012: Silicone implant + ear cartilage graft

4. Surgical Plan


a. Structural Reconstruction

  • Utilize autologous rib cartilage for comprehensive nasal reconstruction.

b. Functional and Aesthetic Corrections

  • Lower the columella to correct depression and improve the nasolabial angle.

  • Adjust nostril symmetry by reshaping the columella and nostrils.

c. Removal of Foreign Substances

  • Remove any remnants of silicone implants and related scar tissue.

d. Functional Improvements

  • Correct septal deviation (septoplasty).

  • Perform radiofrequency turbinate reduction to alleviate nasal congestion.
