Design Consultation: Right Side View, Oblique Side View, and Nostril View Photos
Patient’s Specific Requests:
Achieve a rounded, gently lowered columella
Avoid an excessively elongated columella
Correct asymmetry in nostrils
Maintain the existing silicone implant without altering the nasal bridge or height
Virtual columella augmentation that the patient desires
We were able to accurately understand the ideal nose shape that the patient desires.
2. Surgical Plan and Execution
Procedures Performed:
Columella Adjustment with Donor Rib Cartilage: To achieve the rounded shape and reduce the columella’s visibility, donor rib cartilage was used for precise reshaping.
Right Alar Cartilage Augmentation: Added donor rib cartilage to extend the right alar cartilage, balancing nostril appearance.
Scar Tissue Removal: A triangular scar below the left columella was removed to improve symmetry and smoothness.
Correction of Nostril Asymmetry: Enhanced nostril symmetry to meet the patient’s expectations for a more balanced look.

3. Surgical Outcome
Front View: The nostril asymmetry was significantly improved, creating a more harmonious appearance. The previously prominent right nostril now has a natural, balanced look.
Columella Correction: The columella was subtly lowered, achieving the patient’s preferred rounded shape.
Scar Reduction: The triangular scar below the left columella was successfully removed, enhancing overall symmetry.
