Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.
Today, I want to share a case where a patient, who experienced discomfort and aesthetic dissatisfaction after a thread lift procedure at a traditional Korean medicine clinic, found resolution through surgical correction at our clinic.
1. Initial Findings
The patient presented with:
Mild nasal obstruction symptoms
Residual threads (20–30) from a previous thread lift procedure
Aesthetic dissatisfaction due to a long philtrum and protruding lips
Overall unnatural nose shape

Design Consultation: Frontal View, Left Oblique Side View, and Side View Photos
2. Preoperative Analysis
a. Frontal View:
Bulbous nasal tip
Wide nose
Depressed columella
b. Side View:
Protruding lips
Short nose
Depressed columella
High nasal starting point
c. Nostril View:
Asymmetrical nostrils
Wide and unbalanced shape

Design Consultation: Right Side View, Oblique Side View, and Nostril View Photos
3. Surgical Plan and Execution
a. Removal of Threads and Foreign Material:
Thorough removal of residual threads and any surrounding inflammatory tissue.

Removed Foreign Material
b. Structural and Aesthetic Improvements:
Used septal cartilage and donor rib cartilage to create a natural nasal tip.
Extended and reinforced the columella for balance and harmony with the rest of the face.
c. Functional Corrections:
Addressed deviated nasal septum with septoplasty.
Reduced hypertrophic turbinates using high-frequency laser surgery to improve airflow and alleviate nasal obstruction.

Narrowed the wide nasal tip, creating a sleeker, more natural shape.
Balanced the columella position, restoring symmetry.
b. Side View Enhancements:
Achieved a smoother and more natural nasal line.
Lowered the columella, improving philtrum proportions and minimizing the appearance of protruding lips.
c. Nostril View Corrections:
Corrected nostril asymmetry and balanced the shape.

5. Key Insights
This case highlights the importance of:
Complete Removal of Foreign Material: Residual threads or fillers can cause long-term complications such as inflammation or fibrosis. Thorough removal is essential for a clean surgical foundation.
Comprehensive Approach: By combining functional correction with aesthetic enhancement, the surgery not only improved the patient’s appearance but also addressed nasal obstruction, enhancing the patient’s quality of life.
Use of Reliable Materials: The combination of septal cartilage and donor rib cartilage ensured long-term stability and natural results.
6. Closing Remarks
This case demonstrates how complications from thread lift procedures can be effectively managed with careful surgical planning and execution. For those experiencing discomfort or dissatisfaction after thread lifts or filler treatments, a thorough consultation and tailored surgical approach can provide significant relief and satisfaction.
