Hello, I’m Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’d like to share the case of a 31-year-old male patient who presented with a crooked nose and sunken nasal bridge. This case highlights a customized approach that effectively addressed both functional and aesthetic concerns in rhinoplasty.


1. Patient Background and Requirements


Patient Profile:

  • Age: 31-year-old male

  • Previous Surgery: Hump nose correction with septal cartilage graft (October 2017)

Design Consultation: Frontal View, Left Oblique Side View, and Side View Photos


Main Concerns:

  • Correction of sunken nasal bridge

  • Crooked nose

  • Nostril asymmetry

  • Improvement of chronic rhinitis symptoms

  • Resolution of nasal obstruction

2. Surgical Plan and Approach


To meet the patient’s functional and aesthetic needs, we developed a comprehensive surgical plan:


  • Submucosal Reduction with Radiofrequency Laser: Targeting the inferior turbinates to improve nasal airflow and reduce rhinitis symptoms.

  • Septoplasty: Addressing nasal obstruction and straightening the septum.

  • Septal Extension with Autologous Rib Cartilage: For structural stability and correction of nasal length and tip support.

  • Lateral Osteotomy: Realigning the nasal bones to correct the crooked appearance.

  • Silicone Implant: To elevate the bridge and smooth the uneven contour.


3. Surgical Outcomes

  • The previously uneven nasal bridge now presents a smooth and even contour.

  • The nasolabial angle (angle between the nose and upper lip) was corrected, which improved the appearance of a slightly protruding mouth.

  • The bridge’s bumpy appearance was smoothed, enhancing facial harmony.

  • Correction of the drooping nasal tip for a balanced profile.

  • Improved columella position and symmetry in the nostrils.

Final Thoughts


Today’s case involved a secondary rhinoplasty with a silicone implant to address dissatisfaction with a prior no-implant approach. While implant-free surgery reduces the risk of inflammation or contraction, it may have aesthetic limitations, especially in cases requiring significant structural correction.


Correcting issues such as a crooked nose or asymmetry often requires individualized solutions. In this case, adding a silicone implant was essential to create the desired aesthetic outcome, providing structural support and contour smoothness that a no-implant approach alone could not achieve.
