Correcting Nostril Stenosis and Upturned Nose: A Real Revision Rhinoplasty Case

Today, I’d like to share the story of a patient who came to us with concerns about nostril stenosis and nasal obstruction following multiple surgeries. This case demonstrates the importance of addressing both structural and functional challenges in revision rhinoplasty.

1. Surgical History and Symptoms


  • January 2023: Silicone implant + septal cartilage + ear cartilage graft. (Left nostril began to narrow after 2–3 months.)

  • July 2023: Left nostril expansion surgery + silicone + donated rib cartilage. (Left alar collapse occurred one month later.)

  • Current symptoms: Left nostril obstruction when lying on the right side.


2. Patient Goals


  • Correct pinched appearance of the nose.

  • Improve nasolabial angle.

  • Address upturned nasal tip.

  • Resolve left-side nasal obstruction.

  • Restore symmetry and address columellar depression.

  • Achieve a harmonious straight nasal line that suits the face.

3. Problem Analysis


The left nasal cavity showed significant stenosis caused by:

  • Excessive alar cartilage damage during previous surgeries.

  • Improper suturing techniques, leading to both internal and external nasal valve collapse.


4. Surgical Approach

a. Structural Reconstruction

  • Skin grafts were used to compensate for insufficient tissue.

  • Complete alar cartilage reconstruction.

  • Maximized nasal cavity space to improve airflow.

b. Prioritizing Stability

  • Autologous rib cartilage was used to prevent inflammation.

  • Avoided silicone implants, opting instead for all-natural autologous materials.

  • Ensured all grafts were biocompatible to minimize risks of future complications.

5. Specialist Insights


Nostril stenosis presents unique challenges, especially in cases with a history of inflammation or collapse. The key considerations in this case were:


  • Preventing Inflammation: Inflammation exacerbates stenosis, making meticulous surgical techniques essential.

  • Comprehensive Reconstruction: Restoring both the internal and external nasal valves was critical for functional and aesthetic improvement.

  • Using Autologous Materials: This approach minimized the risk of complications and ensured long-term stability.


While the surgery successfully addressed the structural and functional challenges, the final outcome will depend on the healing process over time.
