Correction of a Wide Nose and Deviated Septum – Autologous Rib Cartilage Case Review

Hello, this is Dr. Cha-Young Kang from Nose Lab Clinic.


Today, I would like to introduce a case in which nasal obstruction and a wide nose were successfully corrected using autologous rib cartilage. This patient had both functional and cosmetic concerns, requiring septal deviation correction and a refined nasal contour.

1. Initial Assessment

  • Mild dorsal hump (humped nose)

  • Droopy nasal tip

  • Prominent projection of the mouth (mild maxillary protrusion)


d. Nostrils (Basal View)

  • Asymmetrical nostrils

  • Septum deviating toward the left


2. Patient’s Goals


  • Improve nasal obstruction

3. Surgical Plan & Execution


a. Functional Improvement

  • Septoplasty to correct deviated septum

  • Submucosal turbinate reduction using radiofrequency laser

  • Restoration of balanced nasal airflow


b. Structural Reconstruction

  • Lateral osteotomy to straighten the nasal deviation

  • Autologous rib cartilage graft for septal extension

  • Bridge augmentation using autologous rib cartilage


c. Aesthetic Refinement

  • Facial fat grafting to balance overall facial harmony

  • Slimming of the nasal contour

  • Controlled nasal height augmentation for a natural look

This case required both functional and aesthetic improvements. Using autologous rib cartilage for septal extension and bridge augmentation, we were able to achieve a naturally contoured, slim nasal structure. Additionally, lateral osteotomy was performed to correct the deviated nose, and submucosal turbinate reduction was conducted to enhance breathing.


The most significant improvements included:

Nasal deviation correction through lateral osteotomy

Structural reinforcement with autologous rib cartilage for longevity

Simultaneous enhancement of nasal breathing and aesthetics
