Hello, I’m Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’d like to discuss the case of a 24-year-old male patient who sought rhinoplasty to address a bulbous and upturned nose, along with nasal obstruction. This example highlights the importance of combining functional improvements with aesthetic balance, especially in male rhinoplasty.


1. Patient Information and Objectives

Design Consultation: Frontal View, Left Side View, and Oblique Side View Photos

Patient Profile:

  • Age: 24, Male


Objectives:

  • Correction of bulbous tip and subtle nasal height enhancement

  • Creation of a balanced nose that complements his facial features


2. Pre-Surgery Analysis

Right Side View, Oblique Side View, and Nostril View Photos

Front View:

  • Noticeable bulbous tip with nasal asymmetry

  • Low nasal bridge

Side View:

  • Low radix (the nasal root between the eyebrows) and a short, upturned nose

  • Mild columellar retraction

Nostril View:

  • Columella tilts slightly to the left, creating an asymmetrical appearance in the nostrils


3. Surgical Plan and Procedure


To address both the aesthetic and functional aspects, we established the following plan:


  • Septoplasty: Correcting deviated septum to improve nasal airflow and overall symmetry

  • Radiofrequency Turbinate Reduction: Using laser to reduce nasal turbinate size for enhanced breathing

  • Septal Extension with Autologous Rib Cartilage: Lengthening and supporting the nose tip to correct the upturned appearance

  • Columella Adjustment: Lowering the columella to address retraction

  • Alar Cartilage Repositioning: Recontouring and securing the alar cartilage to achieve better nostril symmetry

Front View:

  • Short and upturned nose appearance corrected

  • Refined nasal tip with a more defined shape

  • Balanced columella line, improving overall symmetry

Side View:

  • Natural, subtle radix elevation

  • Smooth and straight nasal dorsum, contributing to a more harmonious profile

  • Columella adjustment brings balance to the nose and enhances the lip-to-nose angle, improving facial proportions

Nostril View:

  • Significantly reduced columella tilt

  • Greater symmetry and natural contouring of the nostrils
