Correction of Long Nose and Wide Nasal Bridge: A Revision Rhinoplasty Case

Hello, this is Dr. Cha-Young Kang, the director of Nose Lab Clinic.


Today, I’ll share the story of a patient who sought revision rhinoplasty to address a long nose, wide nasal bridge, and related functional issues. This case highlights the importance of balancing aesthetic and functional goals for a harmonious outcome.


1. Surgical and Procedural History


  • 19 years ago: Nasal filler (later dissolved completely)

  • 20 years ago: Ear cartilage graft + hump nose correction + no implant rhinoplasty

  • 10 years ago: Nasal tip surgery (using septal cartilage)


2. Key Symptoms

  • Scar tissue in the left nasal mucosa causing mucus retention

  • Seasonal nasal congestion

  • Deviated nose

  • Flat and wide nasal bridge

3. Surgical Plan


Aesthetic Goals:

  1. Narrow and sculpt the wide nasal bridge through lateral osteotomy.

  2. Add structure and support using autologous rib cartilage grafts.

  3. Perform alar base lowering to create balance and symmetry.

Functional Goals:

  1. Correct septal deviation for improved airflow.

  2. Use radiofrequency turbinate reduction to alleviate chronic congestion.


Patient’s Preferences:The patient initially considered a no-implant approach but decided to proceed with implant-based augmentation during consultation for a more refined result.


4. Preoperative Analysis

1. Front View:

  • Flat and wide nasal bridge

  • Deviated nasal alignment

  • Thin and under-projected nasal tip

  • Asymmetry in the nostrils

2. Side View:

  • Overall elongated nose

  • Drooping nasal tip

  • Subtle, pointed tip appearance

3. Nasal Base View:

  • Larger right nostril causing asymmetry

  • Flared and wide nasal base
