Correction of Nose Filler Complications and Nasal Congestion_Rhinoplasty Korea


Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’ll share a case from December 2023, where a patient sought help to remove previous nose filler and resolve chronic nasal congestion. This case highlights how we can achieve both aesthetic enhancement and functional improvement with a comprehensive approach.


1. Surgical History and Patient Goals


Treatment History:

  • 2023: Nose filler procedure

  • 2024: Double eyelid surgery

Patient’s Goals:

  • Refine a rounded, wide nose

  • Achieve a naturally elegant appearance with subtle enhancement


2. Preoperative Evaluation

In the initial assessment, the patient’s frontal view showed a broad and rounded nose with slight deviation due to facial asymmetry. From the side, the bridge appeared irregular and overly high at the starting point, creating a less harmonious profile. Additionally, the left nostril was visibly larger than the right, with a slight tilt in the columella.

Upon further testing, the patient exhibited symptoms of nasal congestion. Diagnostic tests at our clinic revealed a deviated septum and hypertrophic rhinitis, which contributed to their breathing issues. A meticulous surgical plan was necessary to address both functional and cosmetic concerns.


3. Surgical Plan and Approach


The surgical plan focused on two aspects:


1. Functional ImprovementTo alleviate nasal congestion, I planned a septoplasty combined with radiofrequency turbinate reduction. These procedures aim to improve airflow by addressing the structural causes of nasal blockage.

2. Aesthetic RefinementThe aesthetic approach involved a multi-step process to enhance the nose’s appearance while preserving functionality:

  • Filler Removal: Completely removed the existing nose filler, followed by using a silicone implant to gently elevate and contour the nasal bridge.

  • Straightening the Septum: Corrected the deviated septum, using autologous rib cartilage to create a natural nasal tip.

  • Adjusting the Columella and Nasolabial Angle: Lowered the columella slightly and adjusted the nasolabial angle to 95 degrees to soften the profile and address the prominent upper lip appearance.

  • Softening the Bridge’s Starting Point: Adjusted the nasal starting point to blend naturally with the forehead and create a feminine contour.

  • Alar Cartilage Adjustment: Repositioned and reshaped the alar cartilages to improve nostril symmetry.


4. Postoperative Outcome

Immediately after surgery, there was a noticeable improvement. The previously rounded nose tip appeared sleek and natural, while the nasolabial angle correction subtly improved the upper lip’s projection. Notably, the nostril asymmetry was significantly reduced, enhancing overall facial balance.

5. Conclusion


Nose fillers may provide temporary enhancements but are often insufficient for long-term, balanced results. True nasal refinement requires a structured, surgical approach, especially when addressing both aesthetics and breathing function. This case underscores the importance of functional and cosmetic harmony in rhinoplasty.
