Low nasal starting point (nasofrontal angle).

Lack of visible columella between the nostrils.
Long philtrum disrupting facial proportions.

d. Nostril View:
Asymmetric nostrils.

3. Nasal Obstruction Findings
CT and nasal endoscopy revealed:
Significant septal deviation.
Hypertrophic inferior turbinates on both sides.
A functional surgery was incorporated into the treatment plan to address these issues.
4. Surgical Process Highlights
During cartilage harvesting, we encountered unusual challenges due to the patient’s prior nasal injuries. Repeated fractures had caused partial calcification of the rib cartilage. Despite these complications, the necessary cartilage was safely and effectively harvested using advanced techniques.
5. Surgical Plan and Execution
a. Functional Improvements:
Septoplasty to correct the deviated septum.
Submucosal reduction of the inferior turbinates using radiofrequency laser for rhinitis treatment.
b. Structural Enhancements:
Extended the nose with septal extension grafts using autologous rib cartilage.
Reinforced the nasal tip with strong support structures.
Lowered the columella and adjusted the nasolabial angle.
Used a silicone implant to create a smooth, straight nasal dorsum.
