1. Preoperative Consultation
This patient visited my clinic to correct a wide, flat and bulbous nose. The nose tip appeared wide and bulbous with thick skin. The area between the eyebrows was flat, and the nose tip looked droopy.
2. Surgery Plan
To correct the bulbous nose appearance, I planned to remove part of the subcutaneous tissue and relocate the alar cartilage, as well as employing the three-dimensional effect technique to increase the height of the nose tip. The three-dimensional technique is the most effective for correcting a bulbous nose. I figured that the he wide nose appearance would also be improved by the elevation of the nose tip, and planned to heighten the flat nose bridge by using a silicone prosthesis.
3. Surgery Content
This was a bulbous nose correction, but since the skin was thick and the nostrils were significantly wide, providing the three-dimensional effect became the most important part in this case. I performed a nasolabial angle correction, especially to improve the droopy nose tip and the protruding mouth appearance, and it had a positive effect on the bulbous appearance in that area as well.

5. General Review
This case was a typical correction of a considerably wide and bulbous nose tip. There are many cases where the bulbous parts of the nose where the flesh itself is big. As there are also many patients with a bulbous nose who want to make their big nose smaller. However, it is not possible to make a big nose small without having to cut the skin. Evidently, to be able to reduce the skin thickness, the soft tissue subcutaneous parts may be removed, which can be expected to slightly reduce the skin, but not as much as one may expect. Therefore, my opinion is that the principle of a bulbous nose surgery is to make the flat and wide nose more voluminous, and heighten the beautiful nose shape to give it a three-dimensional effect, making it look smaller and slimmer.
