[Nose Lab Clinic]
Hello, this is Dr. Cha-Young Kang, the Chief Surgeon at Nose Lab Clinic.
Today, I’m sharing a case where an emergency revision rhinoplasty was necessary due to the imminent exposure of silicone implants.

Design Consultation: Frontal View, Left Side View, and Oblique Side View Photos
1. Surgical History
3 years ago: Dissolving filler injection
2 years ago: Septoplasty + Silicone Implant Surgery
8 months post-surgery: Symptoms of inflammation, treated with medication
2. Initial Diagnosis

Design Consultation: Nostril View, Right Side View, and Oblique Side View Photos
a. Skin Condition
Redness at the tip of the nose
Imminent exposure of silicone implant
Thinning skin at the tip
b. Appearance
Visible nostrils from the frontal view
High radix (starting point of the nose)
Upturned nasal tip
Asymmetry of the nostrils
c. Endoscopic Examination
Left-sided nasal septum deviation
Bilateral hypertrophy of the inferior turbinates
Adhesion of the left nasal mucosa

4. Surgical Procedure
a. Emergency Treatment
Complete removal of the silicone implant
Cleaning of inflamed tissue and removal of damaged tissue

This photo shows the silicone implant extending just beneath the nasal tip skin, indicating that it was on the verge of protruding through.
b. Reconstruction Surgery
Autologous cartilage used to rebuild the nasal tip
Dermal graft placed to reinforce the thinned skin at the tip
Antibiotic treatment plan initiated to prevent further infection
c. Aesthetic Improvement
Correction of the upturned nose and natural refinement of the nasal base
Adjustment of nostrils for symmetry
Natural transition of the nasal bridge and adjustment of the nasolabial angle for a more harmonious result
5. Postoperative Results

Before (Left) / Immediately After (Right)
a. Frontal View
Significant improvement in nostril visibility
Natural nasal bridge height and stable tip position
Symmetry of the nostrils restored

Before (Left) / Immediately After (Right)
b. Side Profile
Softened starting point for a more natural look
Defined nasal tip
Smooth, harmonious side profile lines

Before (Left) / Immediately After (Right)
c. Skin Condition
Improved skin texture at the tip due to dermal graft
Reduced redness and restored stable skin condition

This case was critical as the silicone implant was close to exposing through the skin, creating both aesthetic and functional concerns. The rapid removal of the silicone, along with reconstruction using autologous cartilage and dermal grafting, provided a safe and effective solution.
The thinning of the skin at the nasal tip, caused by previous surgery, was successfully addressed, resulting in both functional and aesthetic improvements.
It is crucial to approach cases involving implants with care, as long-term complications like implant exposure can lead to significant changes in the skin and nasal structure.
