Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic. Today, I’ll be sharing a unique case of revision rhinoplasty that highlights the importance of safe, natural-looking outcomes in nasal surgery.


Surgical History and Patient’s Goals

The patient first underwent rhinoplasty in 2018, which involved the use of septal cartilage, mesh, and osteotomy techniques. However, she came to us seeking a more natural look with a subtle, “non-prominent” nose shape.

2. Surgical Plan: Strategic Approach to Multiple Concerns

To address these complex issues, we developed a detailed surgical plan:


1.Lateral Osteotomy: Narrowing the nasal width

2.Bridge Height Adjustment: Lowering the starting point for balance

3.Tip Refinement:

  • Removal of excess subcutaneous tissue

  • Repositioning alar cartilage

4.Autologous Rib Cartilage Use:

  • Septal reconstruction and extension

  • Subtle dorsal augmentation

3. Surgical Process: Addressing Unexpected Challenges

During surgery, we encountered several unexpected issues:














  • Mesh Removal: We discovered four pieces of mesh (two on each side), which required careful removal to avoid damage to surrounding tissue.

  • Severe Septal Cartilage Damage: Significant damage to the septal cartilage necessitated extensive reconstruction using rib cartilage.

The results were exceptional, aligning perfectly with the patient’s desire for a natural-looking nose:

Comparing pre- and post-surgery photos, the transformation is striking. The previously rounded and wide nasal tip is now refined, while the nasal bridge has a more natural line. The lowered nasion enhances the overall facial proportions, achieving the “subtle, natural nose” the patient envisioned.
