[Nose Lab Clinic]

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’d like to introduce a case of nasal reconstruction and refinement for a patient who underwent orthognathic (double jaw) surgery, which led to septal damage, recurrent rhinitis, and nasal asymmetry. Through a customized approach using donated rib cartilage, we successfully restored both functionality and aesthetics.

2. Initial Diagnosis

🔍 Post-orthognathic septal damage

🔍 Recurrent rhinitis

🔍 Deviated septum

🔍 Asymmetric nostrils

🔍 Wide nasal base

Refined, sophisticated nasal contour (inspired by K-pop star Karina's nose shape)

Nostril symmetry & alar base reduction

Resolution of recurrent rhinitis

4. Surgical Plan & Approach

a. Septal Reconstruction

  • Reconstructed the damaged septum using donated rib cartilage after post-orthognathic nasal trauma.

  • Performed septal extension grafting to enhance nasal tip projection & support.

  • Lateral osteotomy to align the nasal bones and improve symmetry.

b. Functional Enhancement

  • Corrected the deviated septum to restore balanced nasal airflow.

  • Performed radiofrequency turbinate reduction (submucosal resection) to treat hypertrophic rhinitis.

  • Ensured long-term stabilization of nasal function.

c. Aesthetic Refinement

  • Performed alarplasty on the right side to correct congenital nostril asymmetry.

  • Reduced the nasal base width while maintaining natural proportions.

  • Created a sleek, structured "straight bridge line" that harmonizes with the patient's facial features.

5. Post-Surgery Results

This case was particularly challenging due to post-orthognathic nasal trauma and septal damage. Using donated rib cartilage, we restored the nasal framework, allowing for a structurally stable and aesthetically refined outcome.

Key points in this case:

🔹 Precise reconstruction of the compromised septum

🔹 Simultaneous correction of rhinitis & nasal obstruction

🔹 Strategic alar base reduction for improved symmetry

🔹 Seamless integration of a refined, natural nasal bridge contour

At Nose Lab Clinic, we specialize in customized, high-precision procedures that enhance both function and aesthetics.
