Small and upturned nose to a natural shape: dissolving filler removal _ Rhinoplasty Korea

Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today we will introduce the case of a patient who came to our hospital after not being satisfied with the results of a melting filler procedure.

Through this case, we will showcase the limits of the procedures with fillers, and the advantages of structural rhinoplasty.


Preoperative Design Consultation

Filler injections can provide temporary effects, but they have limitations in fundamental structural improvement.

This patient had previously gotten a melting filler procedure one year prior but, the nostril exposure worsen and the nasal bridge line was unclear, so she wanted to fix it. Let's analyze the preoperative condition.

Frontal view:

  • Excessively exposed nostrils

  • A generally small and short nose shape

  • Wide nose tip


Side profile view:

  • Prominent protruding mouth

  • Irregular nasal dorsum

  • Short nose

  • Upturned nose tip causing the nasolabial angle to look unnatural.


45-degree angle view:

  • Blunt and upturned nose tip

  • Unclear nasal bridge shape


Nostril view:

  • Big and rounded nostrils

  • Generally wide nose tip



 


Surgical plan

We came up with the following surgical plan based on the patient's requests and condition:

  • Septal extension graft using autologous rib cartilage.

  • DIssolving filler removal

  • Nasal tip correction and improvement of the nostril exposure.

  • Improvement of the nostril shape.

  • Nasolabial angle correction.

Septal extension graft using autologous costal cartilage is a method that can fundamentally solve the structural problems of a short and upturned nose.

  • Reduced the nostril exposure.

  • Transformed the wide and blunt nose tip into a more refined and slimmer shape.

  • Facial proportion improvement by the adjustment of the lenght of the nose.

  • Changed the appearance of the irregular nose bridge into a smoother shape.

  • Natural-looking descent of the upturned nose which improved the general shape of teh nose.

  • Slight lowering of the columella to correct the nasolabial angle, improving the mouth protrusion.

  • The appearance of the blunt nose tip became more sophisticated and slimmer.

  • The nose bridge shape became clearer and its three-dimensionality was enhanced.

  • The round shape of the nostrils took on a more natural oval shape.

  • By slimming the previously wide nose tip, the appearance of the nose became more harmonious in general.

Overview

This case is a good example of the limitations of melting fillers and the advantages of structural rhinoplasty. Filler injections can provide temporary effects, but they have limitations in fundamental structural improvement. In particular, in this patient's case, the bridge of the nose was able to be raised, but the shape of the nose tip could not be improved.

Septal extension graft using autologous costal cartilage is a method that can fundamentally solve the structural problems of a short and upturned nose. Through this, it is possible to correct a short and upturned nose, and simultaneously solve various problems such as nostril exposure, improvement of the shape of the nose tip, and improvement of the nose bridge line.

By using a closed rhinoplasty approach, we were able to shorten the postoperative recovery period and complete the surgery without leaving a scar, which greatly contributed to increasing patient satisfaction.

It is also important to establish the optimal surgical plan by comprehensively considering the patient's facial structure, skin condition, and previous surgical history.
