Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today we will introduce a particular rhinoplasty case performed to correct a deformity caused by illegal procedures.

The patient suffered acute deformation caused by illegal collagen injections administered 10 years prior.

Nostril view

Frontal view:

  • Abnormally wide glabella.

  • An overall wide and irregular nose tip shape.

  • Prominent nostril asymmetry.


Side profile view:

  • Abnormally high nasal bridge starting point.

  • An overall sagging nose shape.

  • Blunt appearance of the nose tip.


45-degree angle view:

  • An overall large nose.

  • The nose tip bluntness is more evident.


Nostrils:

  • Irregular shape.

  • Severely asymmetrical nostrils.

  • Noticeable scar on the columella.

  • The illegal filler is visible from below the columella.


Patient Medical History:

  • 10 years prior: Unauthorized collage filler procedure.

  • Year 2019: Foreign body removal + tipplasty using ear cartilage and septal cartilage.

  • Present: Nasal congestion symptoms.


Surgical Plan

We came up with the following surgical plan based on the patient's complex condition:

  • Removing the illegally injected foreign bodies (illegal injections) as much as possible.

  • Nose tip cartilage relocation.

  • Septal extension graft using autologous rib cartilage.

  • Septal deviation correction.

  • Turbinate submucosal resection surgery using a high-frequency laser (to improve nasal congestion)

  • Nasal base reduction surgery.

  • The previously wide glabella became more natural-looking.

  • Transformed the previously wide and irregular nose tip into a slimmer and smoother shape.

  • The unnaturally high nasal bridge was lowered.

  • Uprising of the droopy nose tip.

  • Changed the blunt nose shape into a sophisticated semi-curved shape.

  • Improved the appearance of the mouth protrusion throght the nasolabial angle correction.

Overview

This case is a successful correction of a serious nose deformity caused by illegal procedures. It was a complex surgery that safely removed foreign bodies that had been in place for 10 years, while simultaneously improving the function and shape of the nose.

Of particular note is the importance of foreign body removal. Since it is impossible to completely distinguish between illegal fillers and normal tissue, some damage to normal tissue is inevitable. In this process, there may also exist the risk of skin necrosis due to vascular damage.

However, the inner closed rhinoplasty technique performed at Noselab Clinic preserves the nasal artery, which significantly reduces the risk of skin necrosis. This allows us to safely remove foreign substances and reconstruct the nose.

In addition, reconstructive surgery using autologous costal cartilage played a major role in effectively restoring damaged nasal structures. By stably reconstructing the nasal tip cartilage, nasal septum, and bridge of the nose, satisfactory results were obtained both functionally and cosmetically.
